Frozen shoulder, clinically known as adhesive capsulitis or periarthritis shoulder, is a condition characterized by severe pain and progressive stiffness in the shoulder joint.

Overview

The Frozen Shoulder drastically limits the shoulder's range of motion, making everyday activities difficult to perform.In frozen shoulder, the joint capsule becomes severely inflamed, leading to thickening and tightness. This inflammation causes intense pain and limits movement.As the capsule tightens, dense scar tissue forms inside the joint. This restricts the production of lubricating synovial fluid. Because moving the shoulder is painful, patients tend to use the arm less, which only worsens the stiffness and causes the joint to effectively "freeze" in place.

Anatomy

The shoulder is a highly mobile, synovial ball-and-socket joint made up of three primary bones:

  • Humerus: The upper arm bone.

  • Clavicle: The collarbone.

  • Scapula: The shoulder blade.

These bones are enclosed within the joint capsule, a strong connective tissue structure reinforced by articular ligaments. Synovial fluid inside this capsule normally lubricates the joint, allowing the bones to glide smoothly without friction.

Symptoms

The primary symptoms are deep, aching pain (which often worsens at night) and a severe limitation in movement. The condition universally progresses through three distinct stages

Stage 1: Freezing Stage

Characterised by severe, sharp pain with any shoulder movement. Pain is typically worse at night, often disrupting sleep. Range of motion begins to steadily decrease.Generally lasts for 6 to 9 months.

Stage 2: Frozen Stage

Pain may begin to subside or plateau during this phase.Stiffness reaches its peak, making the shoulder noticeably frozen. Activities of daily living (ADLs) become highly restricted. Generally lasts for 4 to 12 months.

Stage 3: Thawing Stage

Pain continues to diminish.Shoulder mobility and range of motion gradually improve. Full recovery can take from several months up to 1 to 2 years.

Causes

While the exact underlying cause of a frozen shoulder is uncertain, several clinical risk factors significantly increase the likelihood of developing the condition.

Risk Factors

  • Age: Most common in individuals between 40 and 60 years old.

  • Biological Sex: Women are more frequently affected than men.

  • Systemic Conditions: People diagnosed with diabetes have a much higher risk and often experience more severe symptoms.

  • Recent Injury or Immobility: Forced immobility following a rotator cuff tear, shoulder fracture, or surgery.

  • Medical History: Higher prevalence in patients with Parkinson's disease, a history of stroke, thyroid disorders, or cardiovascular disease.

Complications

  • Muscle Atrophy (Wasting): Due to the severe pain and prolonged disuse of the arm over months or years, the muscles surrounding the shoulder joint can weaken and physically shrink. 

  • Compensatory Pain (Neck and Back): Because the shoulder joint is locked, patients naturally use their neck, upper back, and shoulder blade muscles to compensate when trying to lift their arm. This abnormal movement pattern can cause secondary muscle spasms, tension headaches, and severe aching in the cervical and thoracic spine.

  • Sleep Deprivation: The intense, aching pain characteristic of the freezing stage is notorious for worsening at night. Chronic sleep disruption can severely impact a patient's overall health and immune system. 

Prevention Tips

While not always preventable, you can significantly lower your risk of developing a frozen shoulder by taking these proactive steps.

  • Early Mobilization: Start gentle range-of-motion exercises as soon as medically cleared after a shoulder injury or surgery to prevent the joint from stiffening.

  • Manage Health Conditions: Keep systemic issues like diabetes and thyroid disorders well-controlled, as they are strong risk factors.

  • Daily Stretching: Routinely stretch your shoulders to maintain joint flexibility and health.

  • Don't Ignore Pain: Seek early physiotherapy for minor shoulder pain. Ignoring it often leads to moving the arm less, which is exactly what triggers the freezing cycle.

Diagnosis

Diagnosis relies on a comprehensive clinical evaluation by a physiotherapist or physician to assess mobility and rule out other underlying shoulder conditions.

  1. Physical Examination

  • Visual Inspection: We'll observe your shoulder and arm for any visible signs of muscle atrophy (wasting) or swelling.

  • Palpation: We'll gently feel around your shoulder joint to identify specific areas of tenderness, inflammation, or structural tightness.

  1. Special Test

  • Range of Motion (ROM) Test: The therapist evaluates both Active ROM (how far you can move the arm yourself) and Passive ROM (how far the therapist can move the arm while your muscles are relaxed). Comparing these motions helps pinpoint the exact limitations in the joint capsule.

  • The I Surrender Test: A clinical diagnostic test where the patient stands against a wall and attempts to place the back of their arm against it in a 90-degree "surrender" position. An inability to touch the wall with the affected arm strongly confirms the presence of a frozen shoulder.

  • Apley's Scratch Test: The test process is, the therapist asks the patient to reach behind his/her lower back, and then behind the head.If it Indicates pain or stiffness or prevents from completing these motions, it indicates restricted rotation and abduction.

  1. Imaging Studies

  • X-ray: Used to rule out bone-related issues like osteoarthritis, bone spurs, or fractures. In a typical frozen shoulder, the X-ray results will appear completely normal.

  • MRI or Ultrasound:  These assess the soft tissues to rule out rotator cuff tears and can directly show the inflamed, thickened joint capsule that causes the condition.

Treatment Options

While a frozen shoulder may eventually resolve on its own over several years, proactive treatment is highly recommended to accelerate recovery and restore function.

Non-Surgical Medical Management

  • Thermotherapy: Application of ice or hot packs to manage localized pain and swelling.

  • Medications: Anti-inflammatory drugs to reduce initial pain and joint inflammation.

  • Injections: Corticosteroid injections administered directly into the joint for rapid pain relief.

Surgical Interventions

Considered primarily when conservative treatments fail.

  • Manipulation Under Anesthesia (MUA): The shoulder is forcefully moved while the patient is sedated to break up the restrictive scar tissue.

  • Shoulder Arthroscopy: Minimally invasive surgery to cut through the tight portions of the joint capsule.

Physiotherapy Management

Physiotherapy is highly recommended and serves as the cornerstone of treating a frozen shoulder. Treatment plans are explicitly tailored to the patient's current stage.

  1. Modalities (For Pain & Inflammation)

  • TENS & Ultrasound Therapy (UST): Utilized to drastically reduce deep-tissue inflammation and manage pain.

  • Heat/Cold Therapy: Applied to reduce joint stiffness and swelling.

  1. Range of Motion (ROM) Exercises

Essential for preventing further scar tissue formation and gently stretching the capsule:

  • Pendular Exercises: Gentle swinging motions to open the joint space.

  • Wand Exercises: Using a stick to assist the affected arm in stretching.

  • Wall Climbing: Slowly walking fingers up a wall to increase vertical reach.

  1. Stretching Exercises

Designed to safely elongate the tight joint capsule:

  • Towel Stretch: Improves internal rotation.

  • Cross-Body Stretch: Stretches the back of the shoulder capsule.

  • Doorway Stretch: Opens the chest and stretches the front of the shoulder.

  1. Strengthening Exercises

Introduced primarily during the Thawing Stage once acute pain subsides:

  • Progressive resistance training using Therabands or light weights to restore full functional strength and address muscle atrophy.
