Supine Terminal Knee Extension (TKE)

The Supine Terminal Knee Extension (TKE) using a foam roller is a targeted strengthening exercise that focuses on contracting the quadriceps at the very end range of knee extension. Because it is performed in a supported position, it is highly effective for safely rehabilitating the knee joint.

How to Perform

Lie flat on your back on the floor or a comfortable mat.

Position the foam roller (or a folded pillow) directly under your target knee so the joint is slightly bent and supported.

Push the back of your knee firmly down into the roller or pillow. Tighten your thigh muscle (quadriceps) and slowly lift your heel off the ground to straighten your leg completely and hold this fully extended position for 10 seconds

Then lower your heel back down to the floor with control.

Quadriceps Femoris

This exercise specifically isolates the Vastus Medialis Oblique (VMO), the teardrop-shaped muscle on the inner side of the knee that is heavily responsible for the last few degrees of extension and kneecap stability.

Tibialis Anterior

shin muscle, engaged when the toes are actively pulled back.

Precautions

  • Maintain Roller Contact: Ensure the back of your knee stays firmly pressed against the foam roller throughout the entire movement. Do not lift the entire leg off the ground.
  • Ankle Dorsiflexion: Pull your toes back toward your shin as you lift your heel. This activates the muscles on the front of the shin and helps fully engage the quadriceps.
  • Mind-Muscle Connection: Actively focus on sending a mental signal to squeeze the inner part of the thigh muscle during the last few degrees of extension.
  • Controlled Movement: Avoid kicking the foot up or using momentum. The lifting and lowering phases should both be deliberate and slow.
