Ankle Pump Exercise
The ankle pump is a foundational, low impact exercise that involves actively moving the ankle joint back and forth. It acts as an active muscle pump to stimulate blood flow in the lower extremities.

How to Perform
1Lie flat on your back on a mat or firm bed.
2Point your toes down and away from your body as far as you can, similar to pushing a gas pedal, to stretch the calf.
3Then pull your toes upward back toward your shins as far as you can. Alternate between these two motions in a smooth, steady rhythm.
🔁 Repeat this exercise 5 times
Muscles Targeted
The calf muscles, active during the downward push
The front shin muscle, active during the upward pull
Helpful For
Essential after orthopedic surgeries like total knee replacements (TKR), total hip replacements (THR), and ankle/foot surgeries.
Used for patients who are hospitalized or immobile for long periods.
Helps manage chronic lower limb edema and poor venous return.
Used in the sub-acute phase of recovery to safely reintroduce movement and flush out localized swelling.
Benefits
- Improves Circulation: Acts as a mechanical pump, pushing venous blood from the lower legs back toward the heart.
- Prevents Blood Clots: Significantly lowers the risk of Deep Vein Thrombosis (DVT), especially during periods of prolonged immobility.
- Reduces Swelling: Helps clear out excess fluid (edema) in the feet, ankles, and lower legs.
- Maintains Mobility: Preserves range of motion in the ankle joint and prevents stiffness.
Precautions
- Avoid Pain: The movement should be entirely pain-free. If there is sharp pain or a sudden pulling sensation, decrease the range of motion.
- Keep it Smooth: Avoid jerky, rapid, or aggressive movements. The pumping should be controlled and steady.
- Don't Hold Your Breath: Breathe normally throughout the exercise; holding your breath can unnecessarily elevate blood pressure.
- Check for Contraindications: If you have an active, diagnosed DVT or severe acute trauma to the ankle (like an unstable fracture), consult a physician before performing this movement.
